Etizolam was found in blood at concentrations of 31 ng/mL and 120 ng/mL, respectively; however, impairment was impossible to determinate or not reported. Etizolam was prescribed or illegally obtained in one case and was taken at supratherapeutic doses. Three patients with psychiatric disorders presented at an outpatient department for etizolam detoxification after exhibiting tolerance and withdrawal. Upon clinical examination, the patient was anxious, but the anxiety resolved, and the patient was discharged the same day . He previously told a friend that at times he took etizolam. The only death reported involved a young man with a history of methamphetamine use found deceased at home.

Side effects include hiccups, dizziness, loss of coordination and drowsiness, along with anterograde amnesia which can be quite pronounced at high doses. Like other benzodiazepines, phenazepam (7-bromo-5-(2-chlorophenyl)-1,3-dihydro-1,4-benzodiazepin-2-one) is composed of a benzene ring fused to a seven-membered 1,4-diazepine ring. This oxygen substitution at R2 is shared with other benzodiazepine drugs with the suffix -azepam. Like most benzodiazepines, phenazepam has a phenyl ring in R5 which is substituted by chlorine in the R2' group. Benzodiazepine drugs contain a benzene ring fused to a diazepine ring, which is a seven membered ring with the two nitrogen constituents located at R1 and R4. Usually, a course of treatment with phenazepam should not normally exceed 2 weeks (in some cases therapy may be prolonged for up to 2 months) due to the risk of drug abuse and dependence.

27 Flualprazolam
These new psychoactive substances (NPS) have the same chemical structure as legal BZD, with an aromatic ring fused to a 1,4-diazepine ring and an aryl group in position R5 34,35,36,37. Due to controlled neuronal inhibition and lower CNS depression risk, BZD rapidly replaced older medications such as barbiturates, meprobamate and chloral hydrate, becoming the most prescribed drug class in the world during the 1970s 11,12. Etizolam, flualprazolam flubromazolam and phenazepam were implicated in the majority of adverse-events, drug offenses and deaths. Non-prohibited benzodiazepines are introduced onto the global drug market and scheduled as rapidly as possible by international authorities.
Learn more about the effects of pharmaceuticals on the environment and how to dispose of unused medicine responsibly. Side effects include dizziness, loss of coordination, drowsiness. Phenazepam is a benzodiazepine drug, which was developed in Soviet Union and now produced in Russia and some CIS countries. It is not a scheduled drug here in the United States or in most of Western Europe.
Addiction
People also do not realize that alcohol at midnight has two-to-three times the psychomotor effects of drinks consumed at 6 pm. Several celebrities have been found with a combination of benzodiazepines and alcohol at their post-mortem. Because of the development of tolerance , some users have to keep increasing their dose to get the same hit or to just try and feel normal again. Benzodiazepines like phenazepam can cause serious psychological and physical addiction. Medical phenazepam will be pure, but there is no way of knowing whether the phenazepam you are buying/taking is pure or not. Taking phenazepam does involve risks.

Most designer BZDs possess a liver metabolism, primarily by oxidative metabolism mediated by the CYP450 family, mainly CYP3A4 . These compounds are sold as tablets, powder, pellets, capsules or blotters, and recently liquids under their own names, at very affordable prices 37, 211, 212. However, beside these BZDs, there are also completely new and recently synthetised NPS BZDs (i.e., pyrazolam, flubromazepam, etc.), some of them were previously research trial compounds whose development did not proceed to clinical use 79, 210. A low dose is mg, a typical dose is mg whilst a strong effect is experienced with mg or more . Bentazepam was firstly seized by the Swedish Police, on 3 March 2014, in Borlänge in a sample of 6 white round tablets. Bentazepam is not currently controlled under the 1971 United Nations Convention on Psychotropic Substances or the 1961 Single Convention on Narcotic Drugs.

He should have called his doctor to help him decrease his Warfarin dose until he was able to go back to his normal green leafy vegetables every day. She should have taken the anti-inflammatory drug for only a short time (one week). The high dose and prolonged use of Naproxen gave her stomach ulcers and made her stomach bleed. Alice had right knee pain and took over-the-counter Naproxen (Aleve)—two tablets, twice a day for two months. Side effects can last for days and sometimes weeks.
Phenazepam Side Effects
Due to the high abuse potential and life-threating consequences of DBZD use, between 2020 and 2021 clonazolam, diclazepam, etizolam, flualprazolam and flubromazolam were listed in Schedule IV of the Convention of Psychotropic Substances of 1971 . The misuse of DBDZ in conjunction with other drug use is a growing and widespread world health and safety concern 47,57,58. About thirty different DBZD have been reported to date to the UNODC Early Warning Advisory (EWA), with the majority of notifications received from European Countries 30,49,50,51,52,53. In 2012 in Finland, pyrazolam, the first true DBZD not approved in any jurisdiction, was identified . They are not strictly considered DBZD since they are approved for medical use in certain countries, but they have been implicated in several drug-related deaths in the United Kingdom between 2012 and 2013 39,47.
Report Of A Massive Oral Overdose
In 36, the cause of death was listed as accidental overdose due to multiple drug toxicity, while in 2 cases they were ruled intentional flualprazolam poisonings. He had previously ingested a large quantity of etizolam tablets. In the third ED admission, diclazepam was the sole drug reported. On 1 September 2014, the UK Focal Point reported that the substance was confirmed after analysis of a blue seized tablet .
- Flubromazepam is a long-lasting BZD, structurally similar to phenazepam, from which it differs due to substitution of a fluorine atom instead of a chlorine atom, and to triazolam and pyrazolam.
- Phenazepam as well as nimetazepam were also detected a few years earlier in Europe in 2007 (UNODC 2013) in seizures by customs and law enforcement agencies in Europe, but at the time, there had been no cases of detection in human samples, either clinical or post-mortem.
- Overall, some of the new BZDs have been historically approved and marketed for use in some countries (i.e., phenazepam); whilst others have been previously investigated and may be found in some patent literature but subsequently not marketed; finally, the remaining ones represent completely new compounds .
- Moreover, bromazolam is the brominated version of the internationally controlled substance alprazolam 115, 153.
- The substance was analytically confirmed by GC-MS, LC-MS (liquid chromatography-mass spectrometry) and NMR analysis by the Swedish National Laboratory of Forensic Sciences 69, 79.
- Like all benzodiazepines, it is legally classified as a C-IV substance in accordance with the analogue act in the U.S. (Flunitrazepam, aka "rohypnol", is dually scheduled.)
Nifoxipam is not currently controlled under the 1971 United Nations Convention on Psychotropic Substances or the 1961 Single Convention on Narcotic Drugs. Flubromazolam is not currently controlled under the 1971 United Nations Convention on Psychotropic Substances or the 1961 Single Convention on Narcotic Drugs. A case-report described a 36-year-old male affected with a schizoaffective disorder, opioid use disorder, seizures, anxiety and a posttraumatic stress disorder who presented to an inpatient facility and reported flubromazolam abuse . Huppertz et al. reported muscle relaxation, sedation, difficulty following and participating in conversation and partial amnesia in a healthy volunteer following intake of 0.5 mg of flubromazolam. Łukasik-Głebocka et al. reported a case of severe intoxication following the intake of flubromazolam in an individual who presented with deep coma, bilateral pinpoint unreactive pupils, acute respiratory failure and hypotension complicated by hypoxic cerebral ischaemia.
- All of these forms of drug abuse can result in addiction.
- In Russia, the indications for phenazepam are epilepsy, anxiety, and sleep disorders.1 Its increase in use for nonmedicinal purposes has been reported in Western Europe in the last couple of years, and its use has spread to the United States, with recorded deaths resulting from its use in Georgia and Louisiana.1,4,5 Some of phenazepam’s concerning side effects include loss of coordination, drowsiness, and amnesia.5 I believe the latter side effect to be particularly concerning due to its potential for use as a date rape drug.
- Data collected in South Scotland from 2010 to 2014 reported 228 cases of phenazepam detection in post-mortem blood samples.
- From a pharmacokinetic perspective, a plasma t ½ of approximately hours is reported 83, 86.
- U.S. Attorney Sally Quillian Yates said, “Selling dangerous drugs is a potentially deadly business whether you do it in person, or via the Internet.
Legal Drugs That Can Kill You
Norfludiazepam is a BZD, structurally related to the internationally controlled substance diazepam, from which it differs due to the addition of a fluoro substituent in the ortho or 2-position of the phenyl ring and by the absence of the methyl group attached to the amide in diazepam. Symptomatology onset occurs minutes after oral intake, effects may last 5-8 hours and after-effects for 1-12 hours after administration . Moreover, bromazolam is the brominated version of the internationally controlled substance alprazolam 115, 153.
Phenazepam Risks
Adinazolam was never FDA approved and never introduced onto the public market; however, it started to emerge as an illegal designer drug in 2015 126,127. Clinical studies revealed that drowsiness and dizziness are commonly observed after oral administration of adinazolam up to 70 mg, resulting in significant amnestic and psychomotor effects at higher doses 123,124,125. A total of 254 cases describing 1 drug offense, 2 self-administration studies, 3 outpatient department admissions, 44 ED admissions, 63 DUID and 141 deaths, reported between 2008 and 2021, are summarized in Table 1. In 2020 amid shortages of classic drugs of abuse following COVID-19 restrictions, some drug users shifted from prescription sedatives to DBZD and novel synthetic opioids (NSO) 63,64,65,66.
5 Diclazepam
A powerful benzodiazepine that provides sedative effects, mainly sold either as a fine white powder or as a liquid. Click on the image below to embark on a brand new journey of drug discovery! An average phenazepam dosage is 0.5 mg 2-3 times daily. It can be used as a premedication before surgery as it augments the effects of anesthetics and reduces anxiety. However, it has no accepted use in the U.S. and is considered an illegal drug there.
Between 2019 and April 2020, DBZD were identified in 48% and 83% of postmortem and DUID cases reported to the UNODC, respectively, with flualprazolam, flubromazolam and etizolam as the most frequently detected substances. Phenazepam magnifies the effects of other substances and can be fatal in small doses, especially when mixed with alcohol and other depressants. The drug, phenazepam, is related to Valium and, while it is not listed as a controlled substance under federal drug laws, its sale for human consumption is prohibited in the United States except for research under strictly regulated terms. The rise of the use of chemical compounds that have physiological and psychological effects but have not been properly tested in clinical trials like licenced medicines is worrying from both a social and medical angle with the drugs having unknown dangers. However, information on the effects of these designer BZDs is largely limited to self-reported experiences coming from online trip reports and limited case-reports and/or case-series 37, 152, 170, 216. Clobromazolam is structurally related to the internationally controlled substances phenazepam, alprazolam and triazolam.
The substance originated by China and the sample was declared as a “sample for research” . Norfludiazepam is not currently controlled under the 1971 United Nations Convention on Psychotropic Substances or the 1961 Single Convention on Narcotic Drugs. Norfludiazepam has a significantly longer half-life compared to flurazepam (up to hours) .
Medical phenazepam will be pure, but there is no way of knowing whether the phenazepam you are buying or taking is pure or not . Phenazepam costs between £18 and £20 for one gram of powder and £12.50 for a 20ml bottle of liquid phenazepam . The benzyl ring of phenazepam is substituted at R7 with a bromine group. It is less toxic than diazepam and in animal studies it appears to produce anxiolytic and anti-agitation properties with limited hypnotic and motor coordination impairing properties. Pinazepam differs from other benzodiazepines in that it has a propargyl group at the N-1 position of the benzodiazepine structure. It possesses anxiolytic, anticonvulsant, sedative and skeletal muscle relaxant properties. Pinazepam (marketed under the brand name Domar and Duna) is a benzodiazepine drug. Watch out for drug interactions.
